This service is offered to patients who have been discharged from a medical facility but still requires continued nursing care. Our skilled and licensed nurses coordinate the care plan of each patient as directed by the primary care physician. They communicate with the patient’s primary doctor and establish a partnership with the family to ensure efficient continuity of care. Under this service we deliver;Wound care and management, Medication administration and management, Post-Operative care such as Catheter care, Management of chronic illnesses such as Diabetes care & Pain management.
